Description

The Palliative Care/ Hospice Nurse is responsible for providing patient/family homecare to meet the needs of patients experiencing life-limiting illnesses. The Palliative Care/ Hospice Nurse works with patients and their family to enrich living- physically, emotionally, socially and spiritually. An interdisciplinary team approach is used to provide comfort and the highest quality of life for patients and their families. The Palliative Care/ Hospice Nurse actively participates in palliative care and hospice program team meetings.


Requirements

Candidates should be highly motivated, posses strong problem solving, communication and pediatric clinical skills. Graduate of an approved school of nursing and unrestricted license as a Registered Nurse in the state of Ohio required. Two years experience in the nursing care of children with life-threatening diseases and knowledge of palliative care, hospice and/or home care and of pain management preferred. Must possess the ability to work independently within a team environment to meet the needs of patients experiencing life-limiting illnesses and their families in the home. Must be able to work closely and effectively with interdisciplinary team. Valid driver's license with clean driving record, proof of automobile insurance and own transportation required.
