Candidate will coordinate and provide clinical care to patients and families who are transitioning from the hospital to their home. This program supports families in caring for their child while creating an environment where they can be as independent as possible. Services provided include: home skilled assessment and patient evaluation, wound dressing care, injections, central line care and a variety of infusions including antibiotics, hydration and TPN. The successful applicant will also have strong communication and teaching skills as this position will also support Home Medical Equipment and Pharmacy areas through teaching of equipment and infusion therapy. Services are rendered on a visit basis and are located in Franklin County and surrounding counties.

Requirements |
Current unrestricted Ohio licensure as a Registered Nurse. Must have infusion experience with central lines and starting peripheral IV's. Requires great utilization experience and experience with assessment, education and infusion skills. Minimum of one-year acute pediatric experience required. Must have reliable transportation to drive between patient homes and a clean driving record. Current CPR required.
