We are an Equal Opportunity/Affirmative Action Employer.RN Care Manager, Provider Relations, Spanish Speaking:An exciting, new opportunity, located in a clinic setting. Managing population health of the clinic and providing care managementto high need patients with multiple chronic conditions or social needs.Monday - Friday, Clinic hours with occasional need to see a patient during extended clinic hours in the evening or on Saturday.1. Develops, assesses, implements, and monitors a comprehensive plan of care for patients within an assigned case load and/or to patients, as assigned, for the team by coordinating an interdisciplinary team approach to service and resource delivery. The plan includes patient problems and goals as well as biophysical, psychosocial, environmental, long-term care, and discharge planning needs.2. Completes a comprehensive, age-appropriate initial assessment based on the patients presenting problems, needs, and strengths and the physician's medical diagnosis. Contacts the physician when additional services such as therapies, social work, or home care are needed.3. Assess existing and potential problem areas; needs and resources of individuals, families, and significant others; family relationships; meaning of illness to client and family; cultural attitudes; and financial impact of illness upon the families4. Conducts ongoing age-appropriate reassessments to determine response to care or services according to organization policy and/or changes in the patient or family status, re-prioritizes care based on new information, and updates the patient's plan of care. Assures the patient and his or her family/caregivers participate in all assessment and planning activities.5. Serves as an advocate and liaison between the patient/family and physician, UniNet team members, community resources, and third party payors as necessary.6. Collaborates with all disciplines to collect, analyze, and monitor clinical outcome data for identified patient populations, which in turn will be used to improve patient care and identify structure or process mechanisms in need of revision. Coordinates, initiates and participates in interdisciplinary patient care conferences and shares patient, family, and physician concerns. Make home visits if necessary.QUALIFICATIONS:Spanish SpeakingExperience:Preferred: At least three (3)to five (5) years previous Care Management, Home Care, or clinic setting preferred.Expertise in care planning, assessments, communication and teaching patient self-management skillsEducation/Licensure:Graduate of accredited school of nursing.Licensure as registered nurse in State of Nebraska or Iowa; Nebraska and Iowa are compact states to must be licensed in the state of residence.Additional Requirements:BLS or BLS instructor certification is required upon hire.Valid Drivers licenseAuto insuranceMust be at least 21 years of age.Knowledge/Skills/Abilities:Requires demonstrated abilities in effective utilization of all home health care services.Requires demonstrated skill in planning and coordination of client care.Knowledge of managed care regulatory process including Medicare and Medicare.Knowledge of Commercial Insurance contracts in relation to covered services.Excellent written and oral communication skills in both English and Spanish.Able to perform multiple responsibilities simultaneously.Able to organize and prioritize.Knowledge of computers.
