Job Description:
Seeking a Home Base Case Manager which will provide housing services to families in Southeastern Ma. Case Management services will have a focus on stabilization, including a mandatory Re- Housing Service plan for each participant. Families will be served for approximately 12-18 months, depending on need. The Home Base Case Manager will provide access to safety net resources.
Home Base will provide:
Outreach to identify families who are at risk of homelessness
Collaboration with the local DHCD Homeless Coordinators
Encourage and promote sustainability and self-sufficiency by providing enough assistance to help stabilize current housing or move to more stable housing and to connect families to other community resources to meet their needs for employment, public assistance, education, and other services.
Engage in Rapid Re-Housing activities utilizing a Housing First model, putting an immediate and primary focus on helping families quickly access sustainable housing thereby avoiding or minimizing shelter stay.
Housing Search and Placement: apartment search utilizing long-standing relationships with landlords and property managers, counseling to tenants, assistance with understanding leases and with securing utilities
This person must exhibit strong assessment skills. Good documentation, organization, and communications skills are a must to adequately handle a caseload and foster connections for clients and community resources.


Requirements

Entrance Requirements:
? Bachelor's in Human Services field preferred and/or Case Management Certification
? Experience working with homelessness
? Experience with SHORE reporting system, a plus.
? Provide families with transportation if needed
? Valid driver's license required
? Bilingual/Bicultural preferred
