Job Description
The RN Care Coordinator works within an interdisciplinary team to manage a defined population of adult patients to improve health outcomes in the management of chronic conditions. The span of the role includes ensuring a patient-centered comprehensive, integrated approach to care across settings. The RN Care Coordinator should have experience in health coaching, geriatric assessment, and motivational interviewing. Ability to work with vulnerable populations and manage behavioral health issues. Ability to use information systems to manage populations of patients and have good problem solving and critical thinking. This role provides ongoing assessment/reassessment of patient using standardized instruments. Delivers care through evidence-based guidelines/protocols/order sets; prompt interventions. Facilitates access to care.
Basic Qualifications:
Bachelor’s Degree in nursing required. Minimum 3 years of experience in oncology nursing.
License or Certification:
Oncology Nursing certification preferred. Must have or be eligible for WI RN license.
