Description

Responsible for analytic initiatives in support of Community & State Accountable Care/Health Home program. Manages day to day processes to integrate clinical data from data warehouse and other sources with the C&S accountable care population registry. Prepares monthly extracts from population registry for use by business teams. Supports implementation of Population Registry in new markets including assisting team with integration with care management platforms and care transitions tools.    

 

Responsibilities including but not limited to:

-  Provide analytical support and expertise when compiling and reporting information
- Evaluate, interpret and analyze data and/or business process
- Develop and present recommendations and alternatives for management and executive review
- May recommend, implement, document, evaluate and/or maintain policies, procedures and controls
- Provide and/or interpret information and data across divisions and departments
- Assume responsibility for data integrity among internal groups and/or between internal and external sources
- Assist in developing department action plans including identification of objectives, goals and strategies
- Develop and implement effective analysis, research and evaluation of accountable care program metrics

- Develop, analyze and monitor new, routine and ad hoc reports using multiple data sources and systems

 

Support Analytic needs of Medical Home/Accountable Care business

-          Maintain databases of hospital ADTs

-          Maintain processes for IPRO risk stratification of practice populations

-          SME for population registry, following up with users, supporting demo and interfacing with DBA

-          Participation on HIE implementation team

-          Work with Business Intelligence to support additional reporting where necessary

-          Maintain analyses related to scorecards at the community level, hospital level and practice level

-          Extract monthly Population Registry data at the practice level

-          Add required fields for outreach and analyses

-          Analyze rates to identify patterns and priorities to address in outreach

-          Manage operational databases as needed




Requirements

Required Qualifications and Experience

- Demonstrated critical thinking and decision making skills

- Excellent oral/written communication skills and presentation skills

- 4+ years collecting, analyzing and presenting data and recommendations to management

- Advanced knowledge and experience using PC software including automated database management systems

- 3 years SQL and/or SAS experience

- Intermediate to advanced working knowledge of relational databases, database structures  - Access

- Advanced Excel

- Math/accounting financial analysis experience preferred.
