We are currently seeking a Bilingual Social Worker- LMSW to join our Lighthouse Hospice team in San Antonio, TX! POSITION SUMMARY The Social Worker provides Medical Social Services to patients/families by a qualified social worker under the direction of a physician and in accordance with the plan of care. ESSENTIAL FUNCTIONS * Assess emotional factors related to terminal illness. * Assist the physician and other IDT/IDG members in recognizing and understanding the social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness. * Assess the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect. * Assess environmental resources and obstacles to maintaining safety. * Participate in development and revision of the plan of care. * Provide social services including: Short-term individual counseling; Crisis intervention; Assist in providing information and preparation of advance directives; Funeral planning issues and transfer or responsibilities regarding fiscal, legal and health care decisions. * Prepare clinical notes. * Identify family dynamics and communication patterns. * Involve the patient/family in the plan of care. * Identify and utilize appropriate community resources and assess patient/family ability to access them. * Participate in in-service programs. * Evaluate patient/family response to psychosocial interventions. * Assess need for counseling related to risk assessment for pathological grief. * Assess special needs related to cultural diversity including communication, space, role of family members and special traditions. * Identify the developmental level of patient/family and obstacles of learning or ability to participate in care of patient. * Address patient/family questions and issues. * Identify obstacles to compliance and assist in understanding goals of interventions. * Identify support systems available to reduce stress and facilitate coping with end of life care. * Evaluate for long-term care when appropriate and assess ability to accept change in level of care. * Communicate psychosocial information to inpatient facility when level of care is changed. * Assist patient/family in assessing financial resources when appropriate. * Identify patient/family needs when discharged or when level of care changes. * Evaluate response to interventions when referred to a community agency and satisfaction with the service provided. * Assess Bereavement needs. * Participate in the QAPI hospice performance improvement program. POSITION QUALIFICATIONS * Accountability - Ability to accept responsibility and account for his/her actions. * Communication - Is available physically and mentally for patient and family communication. Delivers difficult information in honest clear manner. Maintains hope by focusing on palliative care when cure no longer possible. Focuses on helping patient/family live in way meaningful to them. * Emotional Support - Supports patient and family expression of emotional needs. Listens actively, supports as appropriate, and refers to support groups, other patients and families with similar conditions, and/or professionals with expertise in this area. * Ethical - Ability to demonstrate conduct conforming to a set of values and accepted standards. * Honoring Patient Care Wishes - Understands and communicates patient and family end of life care wishes prior to crisis or impending death. Honors wishes as care goals change from cure to comfort care. Only carries out interventions that make a difference for patient comfort and/or recovery. Supports patient and family when treatments are refused. Provides welcoming environment for family to stay with patient. * Psychosocial - Provides an environment to support patient and family expression of psychosocial needs. * Relationship-Family and Communication - Addresses desires and needs for support from family and friends. Determines if there has been a change in family communication. Facilitates family communication of specific issues by structure of interactions. Provides anticipatory guidance for family as they focus on their relationships. This may include reconciliation of relationships. * Reliability - The trait of being dependable and trustworthy. * Team Collaboration - Provides care with a team approach that includes patient and family as integral and essential members of the care team. * Working Under Pressure - Ability to complete assigned tasks under stressful situations. SKILLS & ABILITIES Education : Meets state requirements for position. Experience : Two years of experience in hospice, home health care, or long-term care, with grief counseling specialty preferred. Computer Skills : Basic computer skills Certificates & Licenses : Licensed Social Worker in State where work is performed Other Requirements Valid driver's license and evidence of current auto liability insurance TB test, meet new hire and State requirements for position.Understands the practices and procedures relating to referral of victims of abuse/neglect/mistreatment/exploitation to the appropriate community agencies. Promotes Community education and awareness of hospice services. Understands and maintains professional boundaries in relationship with the patient/family/caregiver.
