El Buens Wallace Mallory Clinic is the medical home for uninsured and underinsured families. The clinic is the only community health provider in Central Texas that by design offers culturally sensitive and bilingual medical services. We are looking for a bilingual (Spanish/English) Diabetes Educator to work collaboratively with our entire healthcare team to provide the highest quality out-patient primary and preventive care as well as acute and chronic care. The health center allows you to work in a patient-centric environment utilizing a modern facility equipped with a Class D pharmacy as well as lab services and state of the art equipment. You will have the ability to get to know your patients, while helping to improve their health status. Job Purpose: The Diabetes Educator provides direct diabetes patient education and develops diabetes patient education activities. Assists in the assessment of the program and its goals. Participates in all quality assurance activities; collects, and compiles data for the Diabetes Education Program. Participates in staff development activities and acts as a liaison with all areas interacting with the program. Essential Functions: 1. Provides individual and/or group instruction to patients with diabetes and other medical conditions. 2. Communicates regularly with the healthcare providers in order to facilitate collaborative planning for follow-up of patients with diabetes. 3. Provides collaborative case management under the supervision of the medical director, while facilitating referrals and medical follow-up of symptomatic patients with diabetes and other medical conditions. 4. Functions as a resource person for other healthcare providers and community members that provide care to persons with diabetes, while participating in lectures, courses, community outreach programs, etc. to maintain state-of-the-art knowledge. (5%) 5. Handles patient, physician, visitor, or administrative inquires promptly, appropriately and tactfully. 6. Willingly and effectively performs other related functions as delegated by the medical director. 7. Participate in developing, assessing and revising diabetes education curriculum and teaching materials as needed, using effective communication skills to assess medical, psychosocial and educational needs of diabetic patients. 8. Develops, implements, and evaluates process improvement and quality assurance plans for department/clinic. 9. Works closely with the management team and nutritionist in developing and coordinating clinical and counseling services related to diabetes management. 10. Participates in the training, orientation, and evaluation of new staff members to the Diabetes Education Program. 11. Performs other duties as assigned. Additional Requirements 1. Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the age-specific and developmental needs of patients served. 2. Appropriately adapts assigned patient assessment, treatment and/or care methods to accommodate the unique physical, psychosocial, cultural, age-specific, and other developmental needs of each patient served. 3. Supports EBSEM mission, vision and values. Demonstrates established customer service behaviors and standards. Treats all clients with courtesy, dignity, respect and professionalism. 4. Appraises his/her performance to identify areas of strength and areas for improvement and to develop a plan for improvement and growth. Skills, Abilities and Other requirements: Excellent Bilingual skills required (English-Spanish) Maintains current CPR certification, CMEs for credentials and any other requirements to maintain licenses and certifications. Strong communication skills, both verbal and written with individuals and groups (presentations). Experience with low income programs and clients, preferred. Working knowledge of Microsoft Office programs. Possess and demonstrate effective research, assessment, program development, and evaluation skills. Ability to instruct others in individual diet, general nutrition principles and food selection and preparation. Knowledge of food service operations, including food preparation, sanitation and procurement, preferred. Ability to communicate effectively with medical, nursing and administrative personnel concerning dietetic care of patients. Ability to set and manage priorities effectively. Education Graduate from an accredited school of professional nursing (BSN preferred) and/or a minimum of a Bachelors degree and completion of an accredited Dietetic Internship required. Registered nurse licensed by the Texas Board of Nursing, and/or registered as a Dietician with the Commission on Dietetic Registration State of Texas, required. Documentation of at least 15 continuing education hours yearly -- specified as being diabetes-specific, diabetes-related, and behavior change self-management education strategies required. Certified Diabetes Educator (CDE) by the National Certification Board of Diabetes Educators strongly preferred. Experience: Formal training in Diabetes Patient Education and/or recent experience for two years in inpatient or outpatient care of diabetes. Documentation of at least 15 continuing education hours yearly specified as being diabetes-specific, diabetes-related, and behavior change self-management education strategies, required.
