Are you a RN who is passionate about making a difference?  Are you sometimes mistaken for the marketing person of your organization?  Share your enthusiasm and talents as a Care Transition Nurse at Heartland Hospice!  Preferred candidates will enjoy initiating and developing professional relationships in hospital and facility environments.  Adeptness to identify, assess and coordinate hospice conversations and admissions with patients, families and providers.  In addition, this individual will partner with the sales team in developing and maintaining referral source relationships.

Successful candidates will have experience in hospice and palliative care or related experience.  Possess excellent communication and organizational skills with a strong aspiration to collaborate with referral sources and team members.  An eagerness to provide education to facilities and the community is a must.  The role may include explaining benefits, assessing eligibility, completing part or all of the admission process and providing daily visit services.

Daily travel in the Northbrook and surrounding areas.  IL RN licensure is required.  Mileage is provided.  This is a full-time weekday role with occasional targeted weekend visits; there are no on-call responsibilities.
