Such functions include the initiation & performance of nursing care through health promotion, supportive or restorative care, disease prevention, diagnosis & treatment of human disease, ailment, pain, injury, deformity, & physical or mental condition using specialized knowledge, judgment, & skill involving the application of biological, physical, social, & behavioral science principles. Essential Functions: - (Adopted from Colorado State Nurse Practice Act CRS 12-38-103) Evaluating health status through the collection & assessment of health data CRS 12-38-103 (10) - Providing therapy & treatment that is supportive & restorative to life & well being either directly to the patient or indirectly through consultation with, delegation to (per Colorado State Board of Nursing rules & regulations for delegation), supervision of, or teaching others - Referring to medical or community agencies per KP guidelines those patients who need further treatment, evaluation, or assistance - Review & monitor therapy & treatment patient plans - Initiate appropriate consultations and/or actions accordingly - facilitates the coordination & integration of care between health care services; manages populations of patients to ensure appropriate utilization of health care resources - Leads health care team by influence & role modeling integrated effective nursing practice, service, innovation & providing outstanding support for physician practices - Patient advocate - Specialty Responsibilities Clinical responsibilities include proactive monitoring, assessment, education & follow-up of patients enrolled in care management to maintain or improve clinical outcomes - Develops individualized nursing care plans, involving patients & their families in self-care practices using motivational & behavioral change modalities - Coordinates care w/ primary providers & the health care team - Additional duties include coordinating components of the Disease Management program at assigned medical offices, teaching diabetes classes, supporting diabetes focus clinics, staff & provider education, planning w/ primary care teams as population management strategies are developed, which may include group visits, outreach strategies for at risk patients, & panel management support to providers - Maintains the population management database - In addition to defined technical requirements, accountable for consistently demonstrating service behaviors & principles defined by the KP Service Quality Credo, the KP Mission as well as specific departmental/organizational initiatives - Also accountable for consistently demonstrating the knowledge, skills, abilities, & behaviors necessary to provide superior & culturally sensitive service to each other, to our members, & to purchasers, contracted providers & vendors
