Description

Accountable Care Practice Analyst works directly with designated physician practice/s and key hospitals as part of the Accountable Care Community partnership to ensure successful implementation of a collaborative population management model which provides individualized, patient-centered, comprehensive care for members. Collaborating with the clinical leaders and hospital teams, the Accountable Care Analyst analyzes population clinical trends, measures outcomes and facilitates communications to ensure interventions improving care are timely, measurable and effective. The Analyst is responsible for analyzing population clinical data and trends, developing work flows to optimize care delivery, and recommending best practices and process improvements. The Accountable Care Analyst is responsible for supporting assigned practices in effective use of the UHC Population Registry and the UHC’s Community Based Care Coordination Tool (CBCCT) to increase their ability to track delivery of evidence based care, and monitor results across the practice population. The Accountable Care Analyst also supports process improvements to assist practices in becoming an NCQA certified Medical Home, if desired.

 

  • Perform analysis and reporting on the data extracts from the CBCCT and report outcomes against goals at monthly JOC meetings with Health Home stakeholders.
  • Improve access to care – use practice level population data to analyze overall visit patterns by clinic overall and assist the Care Coordination Organization (CCO) leadership to implement process improvements to reach mutual goals and improve patient access.
  • Reduce inappropriate ER use – use hospital ER data notifications to analyze trends and work with all community stakeholders to agree on and implement proactive strategies to address issues, and measure impact using a PDSA rapid cycle improvement approach.
  • Reduce avoidable admissions and readmissions – support practices in managing care transitions of all discharges and ensure follow up with PCP or Psychiatrist within 7 days.  Measure and monitor success of outreach and develop strategies to simplify processes and ensure optimal care for patients.
  • Improve care of Health Home/High Risk patients – analyze and report measureable progress against access to care, addressing current care opportunities, and reducing adverse events goals at monthly JOC meetings with Health Home stakeholders.
  • Assist practices in creating work flows to optimize care delivery, introduce best practices improvements, and evaluate outcomes using rapid cycle improvement PDSA methods.  Meet with practice/CCO staff regularly to facilitate progress and address barriers.
  • Prepare and present progress against goals monthly at on-site JOCs with Health Home stakeholders.




Requirements

  • Practice management / operations experience
  • Strong clinical data analytic skills measuring process and outcomes in healthcare
  • Experience managing change initiatives for health care teams
  • Strong skills presenting complex data and outcomes to executive teams
  • Medicaid managed care experience
  • Strong process improvement skills/experience
  • Ability to organize and thrive in a fast-paced environment
  • Access database technical skills
  • Strong interpersonal skills that enable relationship building
  • Effective oral and written communication skills
  • Ability to analyze and monitor results of outcome measures and assist physician practice in creating solutions
  • Strong persuasion skills and demonstrated evidence of innovative thinking and program development to make the health care system work better.
  • Ability to work independently with minimal supervisory oversight.
  • High level of computer literacy with clinical documentation system experience and strong ability to navigate without assistance in multiple computer applications and systems.

 

Additional Preferred:

  • CPHQ certification
  • Six Sigma / Lean certification
  • Master’s Degree in Health Informatics
