Heartland Home Health Care is currently searching for an experienced home care RN to support their team in a Patient Care Coordinator (ADON) capacity. This role allows the right candidate to be responsible for the assessment and evaluation of patients and the supervision, instruction and evaluation of field staff.  Our office is located in Milwaukee and we support patients in Milwaukee, Waukesha, Washington and Ozaukee counties.  We are looking for someone to be available full-time (30-32 hours/week) M-F day shift with flexibility between the hours of 7am-5pm. 

Your problem solving, bedside care, assessment, communication and home care skills/experience will bring success to you in this role. Candidates must demonstrate problem solving and decision making skills. The ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention and family intervention. Demonstrated management ability with an interdisciplinary team and other health care providers.
