The Case Management Manager develops and leads professional clinical and nonclinical staff in required activities which facilitate and support quality, cost efficient outcomes and minimizes fragmentation of health care delivery. The Clinical Manager has experience and expertise in the Case Management process and coaches the staff on the collaborative process of assessment, planning, implementation, coordination, monitoring, and evaluation to address patients holistically. The Clinical Manager supports and coordinates the operations effectiveness across the continuum of onsite and telephonic case management. The Clinical Manager helps staff to understand and demonstrate the ability to integrate the needs of consumers, customers and the business. Overall, the Clinical Manager is held accountable for the ability of the team to meet the business goals and will continually motivate the staff to develop the skills needed to succeed in their roles.
The Clinical Manager will meet with hospital representatives, physician practices and other members of the care system to develop relationships which facilitate communication and coordination of the health care team to maximize services.
