Description

The South End Community Health Center-Community Based Flexible Support Program (CBFS) is seeking an Assistant Team Leader with previous experience in case management or related field. Less experienced candidates may be considered for available Rehabilitative Specialist positions.

The Assistant Team Leader position is responsible for the development of Individual Action Plan (IAP); case management and direct service of individuals served by the program. Responsible for coordinating IAP development for designated individuals, providing support to the team and facilitate ongoing compliance with IAP, making assessments, writing progress notes, entering data, and reviewing progress, developing plan ensuring all team records are maintained in the required timeframe and meet standards, remaining current with all forms of client documentation, reviewing records for each assigned individual on an ongoing basis, integrating clinical understanding into IAP documentation. This position is also responsible for supervising four Rehabilitative Specialists who carry a caseload of 12 clients each, as well as handling on-call responsibilities.

The Rehabilitative Specialist positions have direct client care responsibilities and can manage a caseload of approximately twelve clients. Rehabilitative Specialists conduct assessments of clients' needs and rehabilitative case management. They are also responsible for developing Individualized Action Plans (IAP), and writing progress notes, entering data, and reviewing client progress.


Requirements

Qualifications for Assistant Team Leader: BA/BS Degree in Psychology, Social Services, Human Services, or related major required, three (3) years direct care experience in a behavioral health setting, prior supervisory experience, experience in developing and writing IAP documentation, computer literate in Microsoft Office. Must be able to meet deadlines. Strong attention to details. Bilingual in English and Spanish preferred. Driver's License and access to a motor vehicle is a plus.

Qualifications for Rehabilitative Specialist: BA/BS Degree in Social Services, Human Services or related field preferred, Entry-Level Position. Must be able to meet deadlines. Strong writing and documentation skills required. Experience in developing and writing treatment plans and developing documentation according to Medicaid Rehab Options a plus. Strong computer and coordinating skills. Bilingual in English and Spanish is required. Driver's License and access to a motor vehicle is a plus.
