MLTC Social Work Supervisor (Bi-lingual Spanish) Responsibilities include but not limited to: Uses all available resources to assess members' psychosocial health status and social work needs; this includes making field home visits as needed in order to complete a psychosocial assessment for all new members within the first 30 days of enrollment; Maintain ongoing telephonic communication with members and their caregivers to ensure a safe and appropriate care plan; Work closely with care manager to ensure all Social Work needs are being met; Experience in the development and implementation of new programs; Excellent interpersonal skills, especially the ability to relate well with seniors, their families and community care providers, along with demonstrated ability to handle rapidly changing crisis situations; Experience working in a team environment, exercising independent thinking and problem solving; Must have demonstrated skills in communication, documentation, assessment and social work methods techniques. Qualifications: Master's of Social Work (MSW) NY State LMSW or LCSW; SIFI. 7-10 years post masters experience, including a minimum of 5 years experience in clinical supervision of other clinicians and students. Working knowledge of Medicaid (eligibility screenings, new applications, recertifications, surplus, pooled trust, etc); MLTC, LTHHC, Geriatric, CHHA experience preferred.
