Description

Accountable Care Consultant Analyst works directly with leadership of assigned hospitals and physician practices in each Community, supporting practice transformation from a reactive model of patient care to a proactive Medical Home, analyzing population data to effectively measure, monitor and manage care of patients and present results and outcomes to executive leadership.
 
The Accountable Care Consultant Analyst will be responsible for, but not limited to the following duties:
  • Improving access to care by using practice level population data to analyze overall capacity/demand for appointments and visit patterns by clinic
  • Implementing process improvements to reach mutual goals and improve patient access
  • Analyzing trends and work with all Community stakeholders to agree on and implement proactive strategies to address issues, and measure impact using a PDSA rapid cycle improvement approach
  • Measuring and monitoring success of outreach and develop strategies to simplify processes and ensure optimal care for patients
  • Analyzing and reporting measurable progress against goals at monthly JOC  meetings with practices and hospitals
  • Preparing and presenting progress against goals monthly at on site JOC’s with United/ Practice/hospital executive teams

Requirements:
 
·         Practice management / operations experience
·         Strong clinical data analytic skills measuring process and outcomes in healthcare
·         Strong skills presenting complex data and outcomes to executive teams
·         Access database technical skills, Excel and Relational Databases
·         Ability to analyze and monitor results of outcome measures and assist physician practice in creating solutions
·         Bachelor’s Degree
 
Additional Assets Preferred:
 
·         CPHQ certification
·         Six Sigma / Lean certification
·         Medicaid managed care experience
·         Strong process improvement skills/experience
