Accountable Care Practice Consultant/Analyst works directly with designated physician practice/s and key hospitals as part of the Accountable Care Community partnership to ensure successful implementation of a collaborative population management model which provides individualized, patient-centered, comprehensive care for members. Collaborating with the clinical leaders and hospital teams, the Accountable Care Consultant/Analyst analyzes population clinical trends, measures outcomes and facilitates communications to ensure interventions improving care are timely, measurable and effective. The Consultant/Analyst is responsible for analyzing population clinical data and trends, developing work flows to optimize care delivery, and recommending best practices and process improvements. The Accountable Care Consultant/Analyst is responsible for supporting assigned practices in effective use of the United population registry to increase their ability to track delivery of evidence based care, and monitor results across the practice population. The Accountable Care Consultant also supports process improvements to assist practices in becoming an NCQA certified Medical Home, if desired.

 

 




Requirements

 

 

Basic Requirments:

 

 Undergraduate degree or equivalent experience

 

 one plus years strong clinical data analytic skills measuring process and outcomes in healthcare environment

 

Three plus years experience working with Excel and/or Access spreadsheets in healthcare industry

 

Assets Preferred:


Practice management / operations experience

Experience managing change initiatives for healthcare teams

Strong skills presenting complex data and outcomes to executive teams

Medicaid managed care experience

Strong process improvement skills/experience

Ability to organize and thrive in a fast-paced environment


Strong interpersonal skills that enable relationship building

Effective oral and written communication skills

Ability to analyze and monitor results of outcome measures and assist physician practice in creating solutions

Strong persuasion skills and demonstrated evidence of innovative thinking and program development to make the health care system work better.

Ability to work independently with minimal supervisory oversight.

High level of computer literacy with clinical documentation system experience and strong ability to navigate without assistance in multiple computer applications and systems.
