Bilingual/Spanish Panel Management AssistantDepartment: Managed CareSchedule: Full TimeShift: Day ShiftHours: Monday - FridayJob Family & Level: Job Details: Baseline coordination that includes appointment management, preventive and routine care utilizing a set of tools and processes for population management applied to a primary care panel with the personal clinicians directing proactive care for their patients. Requires high school diploma or equivalent and completion of an accredited Medical Assistant (MA) program plus one year hospital/clinic/medical office experience. Certified as a Medical Assistant by the American Association of Medical Assistants (AAMA), the American Medical Technologists (AMT), or California Certifying Board of Medical Assistants (CCMA). Self driven individual with ability to work independently and exercise good judgment. Also able to work effectively with teams, demonstrating willingness to collaborate to improve services, care management and data processes. Demonstrated knowledge and experience managing patient databases and other computer software programs such as Excel, Outlook, and practice management systems. Experience working with patients in the capacity of health promotion, with knowledge of medical terminology, chronic health conditions, and health coaching. Demonstrated knowledge of chronic care model, fundamentals of health education, motivational interviewing and supporting patients with self assessment goals desired. Ability to protect and maintain strict confidentiality regarding patient information. Demonstrated ability to work and communicate with people from various ethnic, socio-economic, educational and experiential backgrounds. Availability during regular clinic hours including evenings and Saturdays may be necessary. Bilingual English/Spanish required
