Job ID: Company 2233817 GHC Location: MGH Main Campus - Boston Department Care Management-CMHCB Stnd Hrs/Wk 40.00 Shift Day Full/Part Time: Full-Time Regular/Temporary: Regular Job Description The iCMP Team consist of RN Care Managers, Social Workers, Pharmacists and Practice Staff. The CRS is an integrative part of the iCMP team at MGH. The role collaborates to enhance the delivery of patient care services along the continuum of care. The CRS meets patients needs efficiently and expeditiously by continuously improving the patients experience, helping to ensure the institutional standards of high quality patient care; reducing cost and ensuring reimbursement. The position requires a high degree of flexibility, independence and willingness to participate in multiple activities and provide support to all members of the iCMP team. The CRS must have strong communication skills and the ability to converse comfortably with patients and their caregivers, practice staff and program staff and administration. The Community Resource Specialist will work with the iCMP team to ensure enrolled patients are receiving the services they need to achieve an optimum quality of life. Principal Duties and Responsibilities: Research resources and programs available for patients and their caregivers including, but not limited to: * Disabled and elder housing assistance and programs. * Legal, financial and insurance resources. * DME contractors and resources for general home safety equipment. * Support systems for patients and caregivers experiencing grief or bereavement. * Health education material, including a hard and electronic storage system that is easily accessible for program staff. Work collaboratively with iCMP team to support patients. This may include, but is not limited to: * Assist Care Manager and Social Worker with follow up patient support calls * Contact members with memory /language issues to remind about appointments and medication issues; this includes editing, printing, mailing monthly calendars and appointment phone reminders for patients, as needed. * Identify and refer to community services such as transportation, meals on wheels, day programs, volunteers and PCA. * Coordination referrals to internal and external resources, including home care, hospice, etc * Triage services requests and forward them to other program services, as necessary; i.e., Volunteer Escort, PFS * Follow with weekly calls to Skilled Nursing and Rehab Facilities to update Care Managers on progress, as needed * Prepare and submit transportation applications and requests, as requested * Other projects and duties, as needed and specified by iCMP Care Team and/or supervisor * Patient escort as needed Patient Education and Advocacy * Refer patients to resources which would include, prescription drug programs, health & wellness programs, long term care insurance, health care proxy, asset and legal management, government programs, community agencies, public & private organizations, housing options and other services, as appropriate. * Advocate for patient to ensure the members safety, physical, legal & financial well-being. Support their caregivers & quality of life goals and help navigate the healthcare delivery system, Patient Population Served:The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit. Check all that apply. _X Medically Complex Adult (21 - 64) _ X_ Medically Complex Older Adult (65 yrs and up) Qualifications: * High School Diploma Required * Bachelors Degree desired, with demonstrated experience and skills necessary to support the provision of geriatric care management services. * Minimum of 5 years work experience, 2 of which must be related to the Healthcare field * Fluency in Spanish required * Assessment and problem solving skills. * Strong interpersonal skills. * Strong Customer Service skills. * Ability to work independently with minimal supervision. * Goal oriented and accountable in a complex medical environment. * Experience navigating Insurance Networks desired. * Demonstrated organizational and time management skills. * Strong oral and written communication skills. * Good computer experience. Working Conditions: This position requires work and travel between multiple physician practices, community organizations and patients home. Own car required.
