These are Home Health positions working in and around New York City which are a direct result of growth and expansion!Responsibilities:-The Bi-lingual (English/Spanish) Intake Nurse conducts 3-4 enrollment assessments per day to determine clinical eligibility for Managed Long-Term Care Plan-Makes enrollment decisions and develops initial plans of care by utilizing clinical expertise and critical thinking skills-Completes required paperwork and documentation needed to fulfill enrollment requirements-Communicates with families, caregivers, physicians and community providers regarding services and service plans, required documents, and coordinates home visits as neededRequirements:-New York State RN License-Bilingual in either English/Spanish, English/Mandarin, English/Cantonese, or English/Korean-At least 1 year of Home Health experience-Flexibility to cover several Territories in and around NYC-Strong Computer and Communication SkillsIf you feel that you are a great fit for this position call Jeff directly at 610-617-1596 or email your resume to jbrill@judge.com
