Role Summary: The Registered Nurse (RN) Health Advisor is responsible for providing personalized coaching, medical decision support, and information on potential gaps in care to members through telephonic assessment, education, support, and referrals to various health and wellness programs. The RN will use techniques such as motivational interviewing, behavior change, and shared decision making to help people achieve optimal health and well-being. Major Duties A day in the life of a RN Health Advisor may include: * Provides 1:1 telephonic outreach and coaching * Utilization of evidence based medicine standards to support care plan development * Assistance in the identification of member health education needs through health assessment activities * Utilization of health prevention guidelines and general health and wellness strategies to achieve goals in the overall health of members. * Educate and refer members on available health resources and CIGNA Medical Management programs as appropriate * Assesses and incorporate readiness to change into the process of identifying and setting personalized health improvement plans and goals. Provides ongoing support in the support of members in achieving those goals. * Provide decision support where multiple treatment options are available * Work with members on opportunities to close gaps in their overall health status Collaboration with Health Advisor team members such as nutritionist, pharmacist, behavioral clinician, MD and members provider * Empowers members with skills to provide enhanced interaction with their providers. * Collaborates with the team and leadership to identify program areas for improvements and enhancements based on data/experience * Documents interactions and interventions to support ongoing updates and enhancements to ongoing care plan development. * Perform other related duties incidental to the work described herein. Qualifications Ideal candidate will offer: * MUST be fluent in Spanish and English * Demonstrated success in working effectively with individuals with chronic conditions. * Working knowledge of evidence based medicine standards * Working knowledge of motivational interviewing strategies, and active listening. * Working knowledge of health behavior change and its application * Strong computer and software application skills. * Excellent interpersonal skills and the ability to work in a team environment * Demonstrated ability to set priorities and effectively work with individuals on care plan development * A high energy level and excellent written and oral communication skills are essential. * Passion for health improvement of members * A minimum of three (3) years of clinical experience * Diverse Clinical experience preferred * RN with current licensure, Bachelor's degree preferred * Experience with direct member communication ( written and oral) * Experience in telephonic counseling preferred * Experience in health education and wellness coaching EEO Statement CIGNA is an Equal Opportunity Employer Role Summary
