Heartland Hospice has an exciting opportunity for a full-time week day RN Case Manager (very limited on call after hours required) to join our established growing hospice agency servicing patients in the Fox Valley area.

As a RN Case Manager you will supervise and coordinate interdisciplinary care delivery to our home bound hospice patients. In addition you will possess excellent written and verbal communication skills, good problem solving and decision making skills, at least 6 months institutional nursing experience.  Home care or hospice experience is a plus but not needed! We prefer hospice experience but would consider other specialities as well.
