*Monday through Friday, 8:30a to 5:00p, candidate may choose location, either Springfield (VA) or Prince George's Medical Center (MD).
Provides customer focused anticoagulation clinical pharmaceutical care which includes but is not restricted to primary care & anticoagulation services. Communicates w/ providers to influence optimal drug information to patients & health care providers. Participates in patient & provider educational programs to positively impact patient compliance & provider prescribing practices. Oversees & directs pharmacy technician staff in the execution of technical functions.
Essential Functions:
- Supports utilization management initiatives as assigned.
- Assesses clinical outcomes.
- Provides comprehensive pharmaceutical care to members enrolled in clinical pharmacy services including anticoagulation.
- Demonstrates the ability to problem solve.
- Critically analyzes drug literature & lay press to provide drug information to health professionals & members.
- Committed to Target Drug Programs & cost-effective prescribing initiatives.
- Demonstrates clinical pharmacy knowledge & high motivation to seek continuing education to keep abreast of current drug therapies.
- Assesses the drug therapy needs of patients & monitors progress of therapy through interviews, physical assessment, patient education & clinical laboratory monitoring.
- Makes timely decisions about patient drug treatment modalities w/in guidelines & protocols & communicates regularly w/ heath care providers.
- Documents all clinical interventions using appropriate methods.
- Develops & participates in scholarly activities.
- Responsible for maintaining knowledge of policies & procedures & performs in accordance w/ applicable regulatory requirements & accreditation standards as they relate to the assigned department.
- Performs other duties as assigned.
- Assures compliance to all regulatory rules & regulations specific to:
- Drug Enforcement Administration (DEA).
- State Board of Pharmacy.
- HIPAA.

Requirements |
Basic Qualifications:
- 1 year of anticoagulation clinical practical experience required.
- Graduation from an accredited college or school of pharmacy required.
- Licensed to practice pharmacy in assigned jurisdiction, with reciprocation of licensure to remaining jurisdictions (Virginia, Maryland and District of Columbia) completed within 1 year of starting employment.
- Computer literate w/ common word processing, spreadsheet & database computer software & applications required.
- Exceptional interpersonal, verbal, presentation & written communication skills required.
Preferred Qualifications:
- Pharmacy degree w/ clinical pharmacy residency training preferred.
