Works in Cardiovascular Services Department, which includes clinical responsibilities such as proactive monitoring, assessment, education & follow-up w/ patients in person & by phone. Maintains or improves patient health & achieve organizational outcomes. Works w/ patients, their families & the health care team to develop a care plan to improve continuity, quality, & effectiveness of care to members w/ special cardiology needs. Functions as the single point of contact for the patient & family to access advice and/or appointments or discuss questions & concerns. Makes members/patients & their needs a primary focus of one's actions; develops & sustains productive member/patient relationships. Actively seeks information to understand member/patient circumstances, problems, expectations, & needs. Builds rapport & cooperative relationship w/ members/patients. Considers how actions or plans will affect members; responds quickly to meet member/patient needs & resolves problems.
Essential Functions:

- Collects subjective & objective data
- Analyzes reports, & record data
- Uses data to identify health care problems & create plan
- Prioritizes patient's needs
- Triage patients either in person or via telephone & implements proper course of action in accordance w/ established protocol/guideline or in consultation w/ provider
- Conducts telephone calls backs to patients requiring follow-up care
- Evaluates emergency situations & initiates appropriate nursing interventions
- Health teaching & health counseling to patients and/or their families
- Executing delegated medical functions which delivers aspects of care that implement & are consistent w/ the medical plan as prescribed by a licensed or otherwise legally authorized physician or person acting under the physician's delegated authority
- Administers intravenous therapy & other medications
- Providing therapy & treatment that is supportive & restorative to life & well being either directly to the patient or indirectly through consultation with, delegation to (per Colorado State Board of Nursing rules & regulations for delegation), supervision of, or teaching others
- Referring to medical or community agencies per KP guidelines those patients who need further treatment, evaluation, or assistance
- Review & monitor therapy & treatment patient plans
- Initiate appropriate consultations and/or actions accordingly
- facilitates the coordination & integration of care between health care services; manages populations of patients to ensure appropriate utilization of health care resources
- Leads health care team by influence & role modeling integrated effective nursing practice, service, innovation & providing outstanding support for physician practices
- Patient advocate

Requirements |
Basic Qualifications:
:
- Experience working w/ cardiology patients in an inpatient & outpatient setting
- Working knowledge of strong organization & history taking skills
- 12-lead EKG interpretation (or course w/in 1 month of hire)
- Employee Telephone triage experience
- Applicant must be a graduate of board approved RN program & completed a minimum of 750 clock hours of faculty planned clinical experience & guided learning activities which required direct supervision by faculty, associate nursing instructional personnel (ANIP) or preceptor who is physically present or immediately accessible
- ACLS, BCLS required within 90 days of hire
- Currently licensed as a RN in the State of Colorado
- National Provider Identifier (NPI) & Taxonomy code required at time of hire
- Demonstrated customer service skills, customer focus abilities & the ability to understand KP customer needs
-Strong critical thinking and nursing judgment are important skills for this highly autonomous position including ability to effectively conduct telephone triage calls.
-Patient centric, service oriented behavior, strong communication, interpersonal skills, receptiveness for constructive feedback, flexibility, prioritization, and organization skills are essential.
- Floats between departments/facilities/locations.
Preferred Qualifications:
- Bachelor's degree preferred
At least 3 years of recent consistent experience working as an RN in a Cardiology setting (preferably outpatient) is needed.
Device, EP and/or cath lab experience preferred
RN will work supporting the Electrophysiology MDs in cardiology
Strong interest in IT and cardiac medical device technology
Candidate must exhibit a strong interest in arrhythmias & treatments and possess exceptional critical thinking & nursing judgment. Candidate must demonstrate a professional, patient centric service attitude, flexible, organized, and accountable, possess excellent communication/interpersonal skills and ability to prioritize for this highly autonomous position.
Bilingual is a plus.
