As a part of our continued growth, we are searching for
Care Coordinators to join our team providing services to both
Foster Children and
medically complex pediatric patients…
Responsibilities for
Care Coordinators include:
- Function as the Foster Care Program Liaison for the state of Michigan
- Coordinate care with State Foster Care Workers for foster children within the managed care program
- Making outbound calls and in-home visits to assess members' current health status
- Providing psychosocial services and behavioral health needs in the home such as mental health counseling etc
- Identifying gaps or barriers in treatment plans
- Providing patient and family education to assist with self-management
- Interacting with Medical Directors on challenging cases
- Making referrals to outside sources
- Coordinating services as needed (home health, DME, etc)
- Educating family members on disease processes
- Documenting and tracking findings
***This is a field based position requiring the ability to travel within the Southfield and Detroit areas of Michigan***
