Heartland Hospice, a subsidiary of HCR ManorCare, is searching for an experienced Home Health Aide to provide activities of daily living assistance to our patients in their homes. In this role you will work a full-time week day schedule with limited weekend responsibility. This position requires traveling in the Northbrook and surrounding areas.
We require 1 year of C.N.A. experience, valid IL C.N.A. certification, reliable transportation and the desire to care for individuals at end of life.

Educational Requirements:High School diploma or G.E.D.Qualified as a Home Health Aide in the State per State and Federal Regulations. Position Requirements:
Minimum of one year experience as a Home Health or Hospice Aide, or is currently enrolled as a nursing student who has had a minimum of 6 months clinical experience, or has completed a Home Health or Hospice Adie training program or a certified/ state tested nursing assistant (or completion of PCA course- Florida only), as allowable under state specfic guidelines.
