As a part of our continued growth, we are searching for a Disease Management Case Manager to join our team!
 
 
Responsibilities of the Disease Management Case Manager include:
 
  • Patient engagement and retention in specific Disease Management programs
  • Utilize established standards of care to identify and address gaps in care
  • Utilize motivational interviewing techniques for assisting members to move through the stages of behavioral change
  • Work with members to set and achieve SMART goals
  • Coach individuals to make lifestyle changes to reduce risk of disease processes
  • Exercise clinically sound judgment using relevant information, expertise, and decision support tools when interacting with members
  • Achieve productivity standards as established
  • Document clinical information completely, accurately and in a timely manner
  • Educate and empower members to ensure treatment compliance, satisfaction of management of condition, and promote member advocacy

     
***In addition, this schedule for this position requires that you are available to work until 9pm Monday to Thursday. This can be achieved by working 4 ten hour days and a split shift schedule will also be considered***




Requirements

Basic Qualifications :
 
  • Current unrestricted RN Licensure in your state of residence 
  • 5 or more years of Acute Inpatient clinical experience
  • Excellent written and verbal communication skills
  • Proficiency in the following Microsoft computer programs: Word, Excel, and Outlook
 
 
Preferred/Asset Qualifications :
 
  • Disease Case Management experience  
  • Bachelor’s degree in Nursing
  • CCP: Certified Chronic Care provider  
  • Managed Care work experience 
  • Health Coach experience
