Description Patient Care Specialist, Bilingual Spanish - P03873Tracking Code213251-023Job DescriptionProvides bilingual case management services to a caseload of public assistance applicants and recipients whose work readiness is being determined and who may have disabling conditions that impact on employment. Follows participants through a medical service continuum that concludes with an appropriate referral to vocational rehabilitation/welfare to work, wellness programming, or SSI/SSD track.Essential Functions:* Manage the medical service continuum for all participants during the Phase 1 and Phase 2 Bio-psychosocial evaluation. Ensure routing based on medical and social limitations. * Serve as a client advocate during the Phase I and/or Phase II process. * Provide support to the medical team, included, but not limited to Social Worker, Medical Technicians and/or Physicians. * Required Skills* Bachelors Degree preferred or 4 years of experience in a social service-related field.
