Job Description
Clinical Practice - Provides basic therapeutic nursing care according to established standards of practice under direction and supervision of the RN or Provider. Provides care to those patients whose care requirements are routine and standardized with predictable outcomes. The LPN assists the healthcare team to achieve the desired patient outcomes.Quality/Best Practice - Demonstrates knowledge of quality improvement initiatives and participates in unit based performance improvement activities.Professional Development.Assumes responsibility for self development in the life-long learning.Provides direct patient care utilizing the nursing process under the direction and supervision of the RN or provider.Demonstrates knowledge of unit based quality improvement initiatives.Participates in performance improvement initiatives.Reviews diagnostic tests results and reports abnormal results and changes in patient condition to the RN, physician or appropriate healthcare team members in a timely manner.Maintains competencies within practical nursing standards and seeks opportunities to develop professionally.Identifies own learning needs based on self-evaluation and the progress made from the previous year's goals.Collaborates with the supervising RN or provider on the execution of the patient's plan of care.Administers prescribed medications therapies and treatments in accordance with the practical nursing standards.Assists with the orientation of new employees under the supervision of the RN.Seeks opportunities for further increase of practice knowledge.Maintains and reviews patient medical records, notes and physician orders: documents all patients care activities in accordance with the documentation standards.Assists physicians, RNs and health care team members with the data collection, observations, planning, intervention and evaluation of the patient and families care to achieve desired outcomes.Implements components of the plan of care within the LPN scope of practice and competency to promote desired outcomes.Assists with the continuum of care executing the patient's teaching plan under the direction of the RN or provider.Provides compassionate basic nursing care voluntarily assisting others.
Basic Qualifications:
One to two years experience in patient care preferred. Knowledge of continuum of care and community health care services is preferred. Completion of an approved Practical Nurse or Medical Assistant program.
License or Certification:
Basic Life Support (BLS) certification requierd. Licensed Practical Nurse (LPN) License, Medical Assistant Certification (CMA) or Medical Assistant Registration (RMA) in the state of Minnesota required.

Hours/Pay Period: 64
Schedule Details: M-F, 8-5
Weekend Schedule: Occasional weekend hours in Urgent Care.
