Represents & acts as a liaison for the Organization by assisting all (new, returning & existing) members to Kaiser Permanente (KP) facilities & services & most importantly in the selection of a Personal Care Physician (PCP).Assists all members w/their PCP selection to ensure high member satisfaction & member retention for the Organization through on site meetings at Medical Office Buildings (MOBs), phone contacts, & written communications including emails.Conducts data analysis, reporting & presentations for use by Kaiser Permanente Colorado (KPCO) & Colorado Permanente Medical Group (CPMG) management. Makes members/patients & their needs a primary focus of one's actions; develops & sustains productive member/patient relationships. Actively seeks information to understand member/patient circumstances, problems, expectations, & needs. Builds rapport & cooperative relationship w/members/patients, KPCO & CPMG management

Essential Functions:
-Assists all members w/questions/concerns regarding the facility where they will receive their care & services.
-Assist members in their selection of (PCP) in person, by phone calls or e-mails.
-Educate partners in MOBs w/the selection process.
-Provides specific facility & physician information when needed.
-Assist & empanel new KP members in the selection of a primary care physician.
-Ensure Regulatory Compliances by the Federal & State Programs for new members are met.
-Maintaining & updating the HealthTRAC PCP Selection Tool for completion of personal care physician linkage & member outreach attempts & outcomes.
-Working w/Corporate Communications to maintain accurate medical background & biographical information of our physicians/providers for our members.
-Works directly w/all stakeholders in medical facilities to achieve excellence in quality of care & services.
-Generates weekly or monthly panel reports to stakeholders & data analyst when required.
-Conduct presentations to KPCO & CPMG management as required/requested on the PCP selection process & associated data.
-Performs otherrelatedduties as assigned.
In addition to defined technical requirements, accountable for consistently demonstrating service behaviors & principles defined by the Kaiser Permanente Service Quality Credo, the KP Mission as well as specific departmental/organizational initiatives. Also accountable for consistently demonstrating the knowledge, skills, abilities, & behaviors necessary to provide superior & culturally sensitive service to each other, to our members, & to purchasers, contracted providers & vendors.



Requirements

Basic Qualifications:
-A minimum of 1 yr of experience w/data collection, analysis & reporting.
-High school graduation or equivalent
-Demonstrated experience w/excel, PowerPoint, & basic statistics.
-Demonstrated customer service skills & the ability to understand Kaiser Permanente customer needs.
-Demonstrated excellent effective interpersonal verbal & written communication.
-45 wpm typing requirement, w/a 5% or less error rate.
-Must successfully pass intermediate skill level at 60% in excel and PowerPoint.
-Must pass math test at proficient level.
-Must be willing to work occasional weekend or evening hours to meet department needs.


Preferred Qualifications:
- Associates degree preferred
- Experience w/Microsoft Access, Adobe, & Dreamweaver a plus.
- Previous experience in a healthcare customer service environment preferred. Kaiser Permanente experience a plus.
- Bilingual in Spanish preferred.
