Job Description
The RN care coordinator works within the interdisciplinary team to coordinate care for a defined population of patients with complex medical and/or psychosocial issues to improve health outcomes through empowering patients to engage in health seeking behaviors, disease prevention, and self management of chronic conditions. The span of the role includes ensuring a patient-centered comprehensive, integrated approach to care across the settings (ambulatory, inpatient care, family and community).
Basic Qualifications:
Baccalaureate degree in Nursing required. Minimum of three years successful RN work experience required. Strong computer skills required. Demonstrated critical thinking, problem solving, and patient advocacy skills required. Ability to travel frequently to hospitals, skilled nursing facilities, patients’ homes, and other sites where patients receive care (as indicated by patients’ needs) is required.
Other Qualifications:
Masters in nursing or bridged master’s degree in nursing preferred. Work experience in ambulatory care settings preferred. Excellent interpersonal and communication skills, both verbal and written are essential. An ability to work independently is essential. Utilization management, case management and discharge planning experience preferred.
License or Certification:
Current Arizona RN License required. Basic Life Support certification is required upon hire and must be valid for 90 days or greater from start date. Maintains Basic Life Support (BLS) competency.

Hours/Pay Period: Minimum 80 hours per pay period or whatever is applicable
Schedule Details: Monday-Friday, 8:00am-5:00pm
Weekend Schedule: None
Compensation Detail: Education, experience, and tenure may be considered along with internal equity when job offers are extended. The minimum salary every two weeks is approximately $2,368.80, based on a full-time position.
