Responsibilities Conduct intakes, specifically evaluating barriers to care, support systems and health insurance options. Collaborate with the care team, CMP, other staff and patients to improve routine pre-visit planning and post-visit follow-up. Assist in the development of a registry of high risk patients (under the guidance of clinical supervision). Use information systems technologies to reach out to high risk patients to facilitate visits, follow-up, testing, communication and consultation. Developand use standardized documentation tools. Participate in regularly scheduled hospital, practice, team and pod meetings, plus meet with individual providers as needed to develop care plan and systems. Act as a liaison and patient advocate for patients and their families with agencies and health care institutions. Schedule patient appointments through IDX and assist with coordination of care within and outside MGH. Maintain patient education/resource libraries for both locations. Make home visits as needed. Assist with logistics of scheduling group visits. Contribute to quality improvement within Adult Medicine practice. Perform other duties as required. Qualifications HS diploma or equivalent required. At least 3 years of medical office experience required. Must be fluent in Spanish. Proficiency with MS Word, Excel, PowerPoint, Outlook, Clinical Message and LMR. Self-starter with strong organizational skills. Strong attention to detail. Ability to work well both independently as well as part of a team Candidate must obtain certificate from outreach program such as comprehensive outreach education certificate program within first six months of employment.
