Be a part of the Clinical team as a Bilingual RN Care Coach, you will engage our members to develop lifelong well-being and health.

Partners in Primary Care is seeking a Bilingual RN Care Coach to help coordinate and evaluate the management of patients with acute and chronic conditions, as well as patients discharged from acute facilities, to achieve high quality care. You will interact with all levels of nursing and other departments to assure effective utilization of resources that meet the physiological and safety needs of the patient and their families.

*Interact with patients to help transition them from hospital care back to the clinic environment
*Implement risk protocol based on PCP instruction
*Assessing patient conditions and partner in developing and implementing patient plan of care
*Attend daily clinical rounds and present patient information
*Assess patient needs and assist in planning, implementing and evaluating interventions to meet patient and family needs
*Use health coaching skills to engage patients
*Facilitate connections to community support groups and resources
*Follow, coordinate and establish a post discharge plan, including a post hospital needs assessment and primary care follow-up appointment
*Confer with other members of the healthcare team regarding patient care procedures that will directly affect the patient
*Educate and teach patients and families regarding patient disease and procedures that will directly affect patient health
*Maintain and properly log patient follow up calls
*Actively participate in and share professional knowledge as part of a highly collaborate team focused on providing an exceptional customer experience
*Maintain standards for professional nursing practice and demonstrate the ability to use the nursing process in a clinic setting


Role Essentials
*Associate's degree in nursing
*Active, unrestricted Kansas RN License
*English/Spanish bilingual skills fluent in speaking and writing in both English and Spanish
*CPR-BLS from the American Heart Association or ability to obtain prior to hire
*Two plus years of recent hospital patient care experience in cardiac, coronary care, vascular or med-surg, combined with hospice, long term acute care, acute rehab or similar types of care
*Intermediate knowledge of EMRs, Excel, Word and Outlook
*Excellent communication, customer service and telephone skills
*Strong organizational skills and ability to multi-task effectively
*Demonstrated problem solving ability and willingness to take initiative
*This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB

Role Desirables
*General knowledge of clinical quality
*Experience with e-Clinical Works (e-CW)
*Bachelor's Degree in Nursing
