Join us and start doing your life's best work.The Case Manager LPN for the STAR Kids program will manage the care of members in their homes or other community settings by completing in-person health assessments and coordinating patient care, focusing on disease management and keeping members healthy and independent. The Service Coordinator will provide clinical and medical management services, including case management, health assessments, interventions, and discharge planning. This candidate may identify, coordinate or provide appropriate levels of care under the direct supervision of an RN or MD. Work is generally self-directed and not prescribed; this person will work from home when not completing face-to-face patient assessments in the field, so it will be important to function in a less structured work environment. Primary Responsibilities: Engaging members / families in person (in their homes or in a facility) to coordinate services, community resources, and treatment needs Travel in assigned territory as needed to complete member assessments; the rest of the time will be spent working in a home-based office Delivering a holistic approach to coordinated care based on the member's needs using a person centered philosophy Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file Creating, reviewing and revising care plans and focusing on a holistic approach Functioning as an advocate for member Collaborating with the member's PCP to deliver and coordinate necessary services Building relationships with members and their families; assisting them with coordination of health choices Ensuring cases are documented in a timely manner Coordination with other state agencies, community resources and providers


Requirements

Required Qualifications:High School Diploma / GED (or higher) 3+ years recent experience as a case manager, home care, or clinical experience in acute or rehab care settingLVN License, must be current and unrestricted in state of Texas High Speed Internet access in home Strong computer proficiency (Microsoft Word, Microsoft Outlook, and Internet) with the ability to create, copy, edit, send, and saveAccess to reliable transportation that will enable you to travel to client and / or patient sites within a designated area Bilingual (Spanish)Ability to travel up to 50% of the timePreferred Qualifications: Pediatric experience Experience with Medicare and / or Medicaid MDCP (Medically Dependent Children's Program) Case Management experience Managed care experience CCM certification Experience with complex populationsCareers at UnitedHealthcare Community & State.
