Start doing your life's best work.(sm)   WellMed at Northern Hills / Off of Nacogdoches Rd., near O'Connor / HEB Grocery Store - This is a Float position that requires travel to other WellMed locations -  "Mileage reimbursement available"   The Licensed Vocational Nurse, LVN / LPN will provide clinical support and professional nursing care to patients under the direction of physicians, midlevel's and nursing personnel using established standards of care to close the gaps on Quality measures and screenings.   Primary Responsibilities: Coordinates patient care as directed by physicians, company standards and policies including referrals, medication needs, etc. Performs all nursing care duties within the scope of a Licensed Vocational Nurse including administering injections, phlebotomy activities, and Quality screenings Screens patient for chief complaint and history of the condition necessitating the visit and records the information in the patient's medical record Rooms patients according to company protocol Conducts appropriate educational communication to patients Assembles and mails patient records and correspondence to Primary Care Physicians Answers phones, schedules appointments and arranges Comfort Care transportation when needed Maintains certifications/licensure (LVN, CPR, etc.) and quality control standards Performs all other related duties as assigned


Required Qualifications:

High school graduate or GED equivalent
Current state Vocational Nurse License
2+ years of related LVN experience in a clinical setting
Basic knowledge of Microsoft Office programs, including Word and Excel
Intermediate knowledge of medical terminology
ICD-9 and CPT coding
Ability to accurately document information and maintain patient confidentiality
Ability to react calmly and professionally in an emergency situation
Ability to communicate effectively and provide quality customer service
Ability to travel frequently within assigned market
Be flexible and adaptable to change
Current Driver's License and reliable transportation
This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease
Preferred Qualifications:

3+years of related LVN experience in a clinical setting preferred
Bilingual (English/Spanish) preferred
Physical & Mental Requirements:

Ability to lift up to 100 pounds
Ability to push or pull heavy objects using up to 300 pounds of force
Ability to stand for extended periods of time
Ability to use fine motor skills to operate equipment and/or machinery
Ability to properly drive and operate a company vehicle
Ability to receive and comprehend instructions verbally and/or in writing
Ability to use logical reasoning for simple and complex problem solving
Occasionally requires exposure to communicable diseases or bodily fluids
