Behavioral Health Coordinator provide telephonic care coordination and assistance with locating needed clinical and community resources to ensure successful outcomes and maintain a high level of customer service. They are responsible for entering relevant data and clinical information into multiple platforms. Primary Responsibilities Outreach to members or providers who require additional assistance with complex situations Close collaboration with Social Determinant Specialist and other clinical staff, other Optum functional teams and Optum network team Assist with direct linkage to available providers and confirm successful linkage with an appropriate provider Assists enrollees with understanding locating community support services and confirms successful linkage  Assist with provider searches when necessary to meet specific clinical needs, availability issues or geo - access issues Outreach to members who require assistance navigating to an appropriate level of care or provider following an adverse benefit decision Assist with coordination and organization of multidisciplinary rounds Assist with other coordination of care needs of Optum BH staff Demonstrate initiative in achieving individual, team, and organizational goals and objectives Other duties as assigned.



High School Diploma / GED or higher        
2+ year of Social Services or Behavioral Health experience           
Intermediate experience using a computer as the primary tool to complete daily task, including internet navigation and searching while using MS Office applications (Outlook, Excel, Word) effectively to save, send, upload and edit documents         
Working knowledge of community based resources and services           
Must be able to type up to 30 - 40+ wpm, proficiently           
Preferred Qualifications:

Bachelor's degree (or higher) in Social Work, Psychology or other Social Science fields            
Medical / Behavioral setting experience (i.e. hospital, managed care organization, or joint medical / behavioral outpatient practice)  
Bilingual in English and Spanish                              
Soft Skills:

Excellent customer service skills   
Must be able to effectively multi task while utilizing a computer multi-screen and multiple applications           
Strong oral and written communication skills
Excellent interpersonal and problem-solving skills           
Creative problem-solving skills with the ability to use community and network resources to meet members' needs
