If you are located in the greater Jacksonville, FL area, you will have the flexibility to telecommute* as you take on some tough challenges.
Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
Support members with condition education, medication reviews and connections to resources.
Assist members with the transition from a care facility back to their home
Making outbound calls to assess members' current health status
Working with the complex care needs
Identifying gaps or barriers in treatment plans
Providing patient education to assist with self-management
Interacting with Medical Directors on challenging case
Making referrals to outside sources
Coordinating services as needed (home health, DME, etc.)
Educating members on disease processes
Encouraging members to make healthy lifestyle changes
Documenting and tracking findings
Visit members in their homes and/or other settings, including community centers, hospitals or providers' offices
This role involves approximately 25% travel in Jacksonville, FL and youll need to be comfortable dealing with a full range of in-home and facility-based environments. Most of the time youll work autonomously, so the ability to stay focused and motivated is essential.
Current, unrestricted RN license in the state of Florida
Access to reliable transportation
2+ years clinical experience in a hospital, acute care, home health/hospice, direct care or case management
Proficiency with Microsoft Office Suite, with the ability to navigate a Windows environment
Computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
Candidates are required to obtain a CCM within 30 months of employment if you do not already have one
Ability to travel in the greater Jacksonville, FL area approximately 25% of the time
Case management experience
Certification in Case Management
Home care/field based case management
Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
Experience in Home & Community based, Hospice, Home Health or Long Term Care services delivery
If the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
