Primary Responsibilities:Making outbound calls to assess members' current enrollment eligibility for programIdentifying and entering current provider informationProviding patient education on benefits of KRS / KCP ProgramInteracting with Clinical Managers on challenging casesCoordinating care for members by assigning and assisting RNsMaintaining metric expectations for call contacts / enrollments / engagements / qualityDocumenting and tracking findingsRequesting clinical data and data entry function as clinical supportOther duties as requested by management


Required Qualifications:High School Diploma / GED or higher Experience using a Windows PC and learning new application easilyExperience using Microsoft Word (create / edit / send / save documents) and Microsoft Outlook (send / receive emails)Customer Service experienceAbility to work flex shifts, including afternoons, evenings, and weekendsBilingual English and SpanishPreferred Qualifications:Experience in a hospital, physician's office or medical clinic environmentExperience in Chronic Kidney Disease / Dialysis Case Management or clinic experienceTelephonic experienceExperience working in Medicaid and / or Medicare health care and insurance industry, including regulatory and compliance requirementsRenal background preferredExperience with Microsoft ExcelExperience with EMR (Electronic Medical Records)Careers with Optum.
