Primary Responsibilities Engage members telephonically to coordinate services, community resources and treatment needs Deliver a holistic approach to coordinated care based on the enrollee needs Identify early risk factors and conduct ongoing assessments, and document in an electronic file Function as an advocate for members and decrease unnecessary hospitalizations Collaborate with the member's PCP to deliver and coordinate necessary services Expect to spend about 8 hours a day on the telephone helping members from a variety of backgrounds. You'll need to quickly coordinate the right services to meet a full range of behavioral needs, while working in a fast-paced environment.


Requirements

Required Qualifications:

Bachelor's Degree in Social Work
Professional License in good standing for respective discipline
2+ years professional experience in a social work position with geriatric adults/Medicare population
Preferred Qualifications:

Bilingual Spanish
Knowledge of CMS regulations Medicare, Medicaid
Telephonic and/or call center experience
