You have flexibility to work from home as well as provide member care in a direct care setting. This role will require visits to Del Sol Camp LPDSH, El Paso, TX multiple times per week. Primary Responsibilities: Visit members in the hospital to engage in post hospital program Make concerted effort to find members post hospital that are difficult to reach Make initial post hospital call within 3 days of discharge to ensure that discharged member receive the necessary services and resources Identify gaps or barriers in treatment plans Provide patient education to assist with self-management Interact with Medical Directors or member's PCP on challenging cases Coordinate care and services for members Make referrals to outside sources Educate members on disease processes Encourage members to make healthy lifestyle changes Document and tracking findings Prioritize large volume of member activities Adapt to regular process changes as new program evolves Strong organizational skills and multitasking abilities will be keys to success
Current, unrestricted RN license in El Paso, TX
5+ years' experience as an RN in a clinical setting
Bilingual in Spanish
Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
Must have a designated work space and access to install secure high speed Internet via cable / DSL in home
Access to reliable transportation and ability to travel to visit members within El Paso, TX. *Financial assistance for relocation will not be provided
A background in community care or home care
Case Management experience
Certified Case Manager (CCM)
Experience with discharge planning or transitional care services
