Are you looking to help those in need live a more independent, fulfilling life in their own home? HealthStar Home Health is currently hiring a Licensed Social Worker/Qualified Professional to provide services to clients in the Minneapolis, St. Paul area.

As a full-time Licensed Social Worker, you will work with clients and their families to understand the significant psychosocial and emotional factors related to their health problems.

Other duties include:

  • Working with clients and their families to ensure basic human needs, protection, and to assess compliance with the medical treatment plan
  • Prepare clinical progress notes as required. Effectively communicate and coordinate care with all members of the interdisciplinary team.
  • Develop initial and ongoing assessments and social histories to augment existing services being provided to the client.
  • Provide clinical supervision to paraprofessionals (PCAs) per regulatory or payer requirements.
