As a Field-Based LPN Health and Social Services Coordinator UnitedHealth Group, You'll wear many hats and work in a variety of environments. But wherever you happen to find yourself, you'll gain a profound sense of satisfaction knowing you're helping to create a more efficient and effective health care system for everyone.   If you are located within Miami / Miami Dade, Florida, you will have the flexibility to telecommute* as you take on some tough challenges.   Primary Responsibilities: Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care Medication Reconciliation Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services Manage the care plan throughout the continuum of care as a single point of contact Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team Expect to spend about 80% of your time in the field visiting our members in their homes or in long-term care facilities. You'll need to be flexible, adaptable and, above all, patient in all types of situations. Position will be covering the Miami/Miami Dade (Monroe County) Service Delivery area.


Required Qualifications:

HS Diploma
Current, unrestricted LPN license in the state of Florida
 3+ years of experience working within the community health setting in a health care role (or experience as mandated by the state contract)
2+ years of experience working with MS Word, Excel and Outlook
Experience in long-term care, home health, hospice, public health or assisted living
The ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including nursing homes, community centers, hospitals or providers' offices
Preferred Qualifications:

Case management experience including Certification in Case Management
Home care/field based case management
Experience working with the needs of vulnerable populations who have chronic or complex bio- psychosocial needs
Bilingual skills ( English and Spanish)
Experience in Home & Community based or Long Term Care services delivery
A background in managing populations with complex medical or behavioral needs
Experience with electronic charting
Experience with arranging community resources
Prior field based work experience
If the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
