AppleCare Medical Management Group was incorporated in 2003. Over the last 17 years, the multispecialty medical group of independent practices has expanded into new communities, contracted with more hospitals and health plans, and added over 50 new doctors - but the mission has remained the same. Today, AppleCare Medical Management is comprised of hundreds of board-certified private-practice, independent physicians. Collectively, our physicians have been serving the Southeast area of Los Angeles and Northern Orange County for decades, providing their medical expertise and passion to improving the health of our communities.   As a part of our continued growth, we are searching for a new Bilingual Home Support Navigator to join our team!   Schedule: Monday-Friday 8am-5pm 20% office based on-site at St. Francis Medical Center in Lynwood and 80% field based   Primary Responsibilities: Assess member's home environment to identify physical, social, emotional and knowledge barriers to patient care by conducting home visits, gathering assessment information and identifying problems, goals and interventions Collaborates with and supports the Clinical Services team by providing non-clinical duties in the field, including meeting with clients in their homes, nursing homes, shelters, and doctor's offices Provides ongoing face-to-face or telephonic visits with clients to identify barriers to accessing care, and supporting the plan of care developed by PCP and Care Manager Facilitates member's health through advocacy, support with understanding and use of health benefits, assistance in scheduling appointments, development of a support system and reinforcement of self -management and organizational tools


Requirements

Required Qualifications:

High school education or GED
Associates degree or 2+ years of college in human services field
2+ years of experience in community services, preferably a healthcare environment
2+ years of experience working with older and disabled individuals and their families
Bilingual Spanish Speaking
Proficient in MS Office
Must have access to reliable transportation and ability to travel up to 80% local travel
Working knowledge of medical terminology and abbreviations
 
Preferred Qualifications:

Bachelor degree
Medicare/Medi-Cal experience
Thorough knowledge of community resources, in-home operations and community-based service waivers for Medi-cal beneficiaries
Knowledge of utilization coverage guidelines, coordination of benefits, and   coordination of care processes provided throughout the healthcare continuum
