This position will require active and unrestricted Nursing licensure in multiple US States. Selected candidate must be willing and able to obtain and maintain multiple state licensure. (Application fees and filing costs paid by UHG)
If you are located in Montgomery County, TX, you will have the flexibility to telecommute* as you take on some tough challenges.
*** Telecommuter position with some ride-alongs for audits. Travel will be up to 20% within the Montgomery County, TX area. ***
Primary Responsibilities:
  • Provide a complete continuum of quality care through close communication with members via on-phone interaction
  • Support members with condition education, medication reviews and connection to resources
  • Assist members with the transition from a care facility back to their home
  • Work with members who were hospitalized and are at risk for re-admission
  • Conduct post-hospitalization assessments telephonically
  • Identify member needs and formulate plans of care

Required Qualifications:
  • 3+ years of clinical RN experience in acute care or home health OR case management experience
  • Multiple state license (in addition to Compact License if applicable) or ability to obtain multiple state nursing license
  • RN license in the state of Texas
  • Willing to do up to 20% travel in the Montgomery County market
  • Willingness and ability to obtain CCM within 30 months of hire
  • Ability to talk and type simultaneously
  • Computer/typing proficiency with Word, Outlook, Excel, and Internet
Preferred Qualifications:
  • CCM
  • Bilingual (Spanish)
  • Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
  • Ability to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others
  • Medicaid and/or managed care experience
