The RN Clinical Patient Educator acts as an educational resource, and advocate for patients with high-risk diagnoses such as Diabetes, CHF, IHD and COPD/Asthma and or patients with co-morbidities through the market. The Educator also conducts wellness and prevention classes for all patients in the market. This position interacts and collaborates with a multidisciplinary care team, which includes physicians, nurses, case managers, social workers and other educators to develop educational strategies and care management interventions that will assist in maximizing patient outcomes. This position supports Clinical Programs activities by providing staff training related to the implementation of special projects and grant programs.

Primary Responsibilities:

  • Conducts classes at various clinics on chronic disease states to include but not limited to CHF, IHD, COPD and Diabetes and health and wellness topics
  • Assist in providing patient empowerment through use of motivation, supportive skills, problem solving, and self-management goal setting

  • Collaborates with multidisciplinary care team including physicians, nurses, case managers, social workers and other educators/health coaches to identify and provide effective education techniques for plan member

  • Evaluates effectiveness of teaching methods and provides feedback, recommendations and corrective action plans to the multidisciplinary care team for ensure patient progression toward goals

  • Conducts learning needs assessments to identify patient knowledge deficits and utilizes effective educational techniques

  • Ensures that all educational content is updated to reflect changes in standards of care, and formulary and procedures changes

  • Ensures all new initiatives, policies and procedures, guidelines or other necessary updates are incorporated into patient educational program

  • Manages travel and class preparation in an efficient and effective manner utilizing time management skills to facilitate the total work process

  • Participates in the development of appropriate QI processes, establishing and monitoring indicators

  • Conducts staff training related to the implementation of special projects and grant programs

  • Assists in the implementation of those programs as directed by the Clinical Program Manager

Required Qualifications:

  • Registered Nurse with a current license to practice in the state of Texas
  • Bachelor of Science in Nursing (BSN) or ADN with 5+ years documented nursing experience

  • Diverse clinical experience with 3 or more years’ experience in caring for the acutely ill patients with multiple disease conditions

  • Possess planning, organizing, conflict resolution, negotiating and interpersonal skills

  • Proficient in PC software computer skills such as Microsoft Word, Excel, Power Point and other departmental specific programs/applications

  • Bilingual language skills (Spanish) – speaking, reading and writing

  • This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor’s diagnosis of disease
