The role of the Palliative Care (PC) Nurse Practitioner is that of advanced nursing care provider, clinical consultant, and patient/family educator in the implementation and coordination of multi-disciplinary supportive care of the patient with complex needs within the outpatient, inpatient, and patients home settings. In this role, the PC nurse practitioner, in collaboration with the PC physician and PC nurse navigator, will coordinate and provide complex palliative care for at-risk patients, patients requiring symptom management and supportive care, and developing goals of care and treatment plans. The PC Nurse Practitioner utilizes both an interdisciplinary and multidisciplinary approach to provide seamless care delivery, integrating a wide range of providers and services, such as, pain management, social work, financial advocacy, spiritual care, specialty navigators, nursing care and medical management. The PC Nurse Practitioner is expected to actively participate in the health systems efforts to provide internal and external coordination of care that will add benefit and comfort to the PC patients quality of life. Palliative care patient management requires building long-term relationships and networking for ongoing coordinated care to improve the overall quality of life of the population served.

Expert Clinician and Educator:

Leads the development and implementation of standards of care related to the management and evaluation of palliative care patients.

Collaborates in the development of multidisciplinary goals of care planning for palliative care patients.

Serve as an expert clinical resource to nursing and medical staff.

Responds to and is directly responsible for clinical consultations in outpatient, inpatient, and patients home settings.

Defines care standards in collaboration with physicians and care team members for symptom management and/or end-of- life care.

Performs an independent, comprehensive physical examination, formulates a medical diagnosis and treatment plan for palliative care patients.

Participates in conferences, medical and nursing grand rounds and M&M conferences.

Presents recommendations to the patient, family members, primary care physician and other members of the patients multidisciplinary care team.

Provides expert pain management to seriously ill patients including non-pharmaceutical approaches, safe and effective prescribing of opioids in compliance with state and federal regulations, knowledge of dose equivalents and converting one opioid to another, management of patients with patient controlled analgesia in all practice settings including hospital, ambulatory and the home are required.


Collaborates with the other care team members to develop criteria and standards by which quality of care can be measured, and assures that ongoing monitoring and clinical evaluation continually meet or exceed benchmarks.

Maintains current, up-to-date knowledge of palliative care based on Advanced Nursing Practice standards and Palliative Care credentialing organizations.

Develops and implements a survey for primary care providers/specialists and patients/family members to determine outcomes and satisfaction with the palliative care program.

Develops templates and clinical pathways based on evidenced based guidelines and literature.

Maintains compliance with state, federal, and JCAHO requirements.


Seeks opportunities to participate in clinical and nursing research projects.

Seeks opportunities to publish outcomes of symptom management and palliative care- related professional contributions.

Change Agent/ Program Development

Participates in long-term planning activities and assists with the development/implementation of

new programs.

Actively participates in the clinical development of the AAMC Palliative Care Program.

Develops required documentation tools in EPIC with the IS and professional team.

Serve as liaison and coordinates care with the nurse navigators to improve communication across

the care continuum and provides networking with community resources for supportive services.

Serve as community spokesperson for presenting the AAMC Palliative Care Program to internal and external audiences.

Required Minimum Education:

Masters Degree from an accredited Nurse Practitioner Program.

Required Minimum Experience:

Three years of experience as a Nurse Practitioner.

Three years nursing experience in outpatient and/or inpatient environment, or community care setting.

Able to demonstrate direct involvement in a multidisciplinary team caring for palliative care patients.

Required License/Certification:

Palliative Care, Hospice or Oncology Advanced Practice (APRN) certification must be obtained within one year of hire.

Certificate of completion of CAPC's Pain Management Curriculum must be completed within the first 6 months of employment.

Light Work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
