As an active participant of the patient centered medical home team, the LPN Care Manager engages in a collaborative process with members of a multidisciplinary team which may include a Physician, Nurse Practitioner, RN Care Manager, Social Worker, Pharmacist, Clinic Manager, and Medical Assistant. The team assesses, plans, implements, coordinates, monitors and evaluates options and services to meet a patient’s health needs through communication and utilization of available resources to promote efficient, quality outcomes. The LPN Care Manager provides quality nursing care to patients and assists the Physician and Nurse Practitioner in the clinical setting.

The LPN Care Manager collaborates with the primary care provider and other members of the care team and supports the patient by providing routine nursing care, patient education, and self-management support. Addressing the total patient, inclusive of medical, psychosocial, behavioral, cultural, and spiritual needs is important for successful patient management. In addition, the LPN Care Manager provides telephonic and face-to-face follow-up for patients with complex and/or high-risk conditions, assisting in care management and care transitions.

Principal Responsibilities:

  • Functions in a direct patient care role.

  • Supports RN Care Manager with telephonic and face-to-face management of patients’ chronic conditions including behavioral health and social issues

  • Involves the individual patient and caregiver, as appropriate, in decision-making

  • Facilitates communication and coordination among members of the healthcare team

  • Conducts pre-visit planning by reviewing patient’s medical records to determine preventive care and treatment needs prior to scheduled appointment

  • Manages incoming patient calls and messages by communicating with patients, family members, and care team to ensure that the patient’s needs are met and appropriately documented in the telephone encounter in the electronic medical record.

  • Provides patient care to include patient assessment/triage prior to physician’s examination, serves as patient advocate/liaison by answering questions related to care and treatment, patient education and self-management support, implements physician

    orders (injections, prescription refills, diagnostic tests, referrals, etc.) and assists physician or nurse practitioner with procedures.

  • Performs clinic/administrative tasks as directed by clinic manager

  • Triages phone calls to the appropriate department

  • Performs other job-related duties as requested

Primary Responsibilities:

  • High School diploma or GED

  • Unrestricted Licensed Practical Nurse in Louisiana

  • 1+ years of experience with Microsoft Word, Outlook, Excel (include spreadsheet use) and internet

  • 1+ years of experience with case management or educating patients

Required Qualifications:
  • Bilingual in Spanish and English

  • Experience with EMR (Electronic Medical Records)

  • Home health experience
