Health Educator
Community Impact - Steamboat Springs, Colorado

To advance the Health Partnership's Community Health Impact Program, we are seeking an organized, self-starter, with excellent communication skills to serve as our Health Educator to build community capacity surrounding environmental factors that impact health. Develop and implement strategies to improve the health of individuals and communities through health promotion, screenings and referrals.

Supervision Received: The Health Educator is based out of the Routt or Moffat County office and supervised by the Program Manager.

General Requirements & Qualifications:

The Health Partnership is seeking candidates who meet or exceed the following qualifications:
Bachelor's degree in public health, social work/human services, communication, or relevant experience required.
Ability to communicate effectively with diverse audiences including community members, clinical partners, health and human service agencies.
Experience developing and providing strategic communications including the creation of written and oral content
Experience in community outreach and/or recruitment, desired
High levels of organizational skills with a focus on detail and follow through
Passion for social justice and treating all with equal respect and dignity
Competent with Microsoft Office, Dropbox, Box and Google
Familiarity with social media, Wordpress and Mailchimp, desired
Possess a Colorado driverâs license and have access to a reliable vehicle and be willing to travel
Must be able to lift office equipment up to 20-pounds and climb stairs at site/client meetings.

Key Job Responsibilities:
Support the facilitation of Advisory Consortium to advance population health and increase community capacity to advance the social determinants of health.
Plan and deliver appropriate trainings.
Recruit and support clinical partners with screening for social determinants of health.
Through education and support, increase screening volumes throughout the region.
Participate in training and technical assistance activities with partners.
Increase outreach and engagement opportunities to strengthen community and clinical partner referral pathways for care coordination and increase "opt-in" rates.
In Partnership with Managing Supervisor: provide support for strategies and approaches to facilitate open communication to keep project partners apprised of and engaged with progress, opportunities, and resources.
Complete monthly reporting and requirements for Accountable Health Communities Model activities.
All other duties as assigned.

Engage in cross-organization efforts, connecting project work to the broader Partnership.
Share unique skills and expertise with NCCHP team.

.8FTE (32/hours per week) plus benefits;
