Description

Service Coordinator/Case Manager Families First is looking for a Service Coordinator to join our dynamic leadership team. The caseload of 12-14 youth/adults with disabilities, will primarily be in the Deerfield Valley and Bennington, VT. An understanding of Vermont's system of care for people with disabilities, a dedication to promoting the civil rights of marginalized people and a passion for creating inclusive communities are necessary as well as a Bachelor's degree. Familiarity with Electronic Health Record reporting, team leadership skills and a sense of humor are also important.
