We are seeking dedicated professionals to staff our quality programs.

Per Diem – Direct Care $12.00/hr                                              Closing Date: 05/10/2020

Fill in staff needed to support service recipients in all of our programs on an as-needed basis.

First/Second shift Duties include, but are not limited to, teaching skills such a cooking, menu planning, laundry, providing support for ADL skill development, grocery shopping, and documentation of services provided into an ERMS. 

Third shift duties include, but are not limited to, light housekeeping, regular bed checks, monitoring mental and physical health of members, medication cueing when necessary, and documentation of member services with our electronic medical records system (EMRS). CNA/HHA preferred

Hours: Varied to include evenings, overnights and weekends.

Duties and Responsibilities:

  • Assist members with participation in recreational, social, vocational, residential, and community events.
  • Assist members in performing daily living skills and meeting daily living needs such as shopping, laundry, housekeeping, transportation, etc.
  • Assist with the implementation of goal activities as defined on the individual’s Service Plan.
  • Provide crisis intervention supports as needed.
  • Provide emotional support to members.
  • Monitor and document member’s physical and mental health and report relevant information to the appropriate staff.
  • Provide conditions that maximize safety for staff and program participants.
  • Provide coverage as needed at any of the Agency sites per Agency need.

  • Engage in continuous concerted efforts to access, obtain, respond to pertinent agency information/communications, and disseminate as appropriate.
  • Provide working conditions that maximize safety for staff and program participants.
  • Maintain CPR/FA Certification
  • Monitor and Document interactions and observations.
  • Other job-related duties as assigned.
  • Perform duties while demonstrating Agency Core Competencies and values

Education and Experience:

  • High School Diploma or equivalent
  • Experience in the provision of mental health preferred.
  • Valid driver’s license
